Scuba Diving Forms

• Medical Information w/Physician Approval 
  (Needed if you put a "YES" in any box of the Medical Information in the student record file.
• Continuing EducationRelease
  (Scuba Review, Advanced Open Water, Specialties, Rescue Diver - this is the form you will need to complete)
• Discover Scuba Diving Registration Form & Liabilty Release
  (This is the form you will need to have filled out if you are attending any of our Discover Scuba events.)
• Youth Responsibility Release
  Are you under 15 years old?  We need this signed by you and your parents or guardians before you can take a class with us.
• Master Scuba Diver Application
  The application you submit to us for your Master Scuba Diver Rating.
• Certification Card Replacement -  Lost your PADI C-Card and need a replacement? Seatasea can process your replacement card on-line.  Simply complete the information on-line and email us a picture of yourself.  Cost is $35+ IA Sales Tax 7% if you live in Iowa. 
• Seateasa Scuba Certification Picture Identification Card On-Line Processing Form
  If you prefer to mail this in.  But we will need a picture.
