Administrative, Applications and Releases

- PADI  Medical Information w/Physician Approval  (Needed if you put a "YES" in any box of the Medical Information in the student record file.

- PADI Master Scuba Diver Application

- Discover Scuba Diving Form & Liabilty Release (This is the form you will need to have filled out if you are attending any of our Discover Scuba events.)

- Certification Card Replacement -  Lost your PADI C-Card and need a replacement? Seatasea can process your replacement card on-line.  Simply complete the information on-line and email us a picture of yourself.  Cost is $35+ IA Sales Tax 7% if you live in Iowa.
